What our patients say
Sometimes I get so busy that I would forget to eat lunch, and then get headaches. I was curious if it had to do something with blood sugar. The CGM data was very insightful.

What our patients say
I joined because I was interested in weight loss and understanding my pre-diabetes risk. The CGM data helped me monitor my blood sugar and gave me excellent control over keeping my blood sugar flat.

What our patients say
A CGM is so much better than finger sticks. I realized I was having hypoglycemia overnight and spiking a lot in other situations. I used to eat dinner late, now I eat earlier and I'm incorporating more exercise.

What our patients say
I've been trying to stick to a super low-carb diet but it doesn't feel great and it's not sustainable. The CGM and my primary care doctor have really helped me find the right choices for me.

Frequently asked questions

  • Who is eligible?
People between the ages of 18 and 65 who live in California and whose healthcare is not covered by Medicare or some other government programs are eligible to apply for the assessment. If deemed eligible, a provider will write a prescription for a CGM.
  • What does this health assessment cost?
The initial cost of the assessment is $50. For this price, you will receive a virtual appointment that will provide you with an introduction to the program, a prescription for a CGM and access to your blood sugar data through the connected app. The cost of the CGM will vary depending on your insurance. You may wish to book a follow-up appointment with one of our primary care providers after reviewing your initial blood sugar data. The cost of this visit will also vary, depending on your insurance.

You may be eligible for a free voucher for a CGM device if you’re a first-time client with Abbott, which you can use at your local pharmacy or Alto Pharmacy.
  • What if I don’t have insurance?
No insurance? No problem. We believe everyone deserves to understand their metabolic health and gauge their risks for future complications. That’s why we offer options for self-pay and are committed to cost transparency, so there are no surprises along the way.
  • Why should I care about my metabolic health?
Understanding your metabolic health is one of the fastest ways to reach your health goals and determine your risk for future health complications.

Using a CGM device will help you discover how your blood sugar responds to your meals, physical activity, and other lifestyle factors in real time. It’s a painless, discreet way to gather hundreds of glucose readings per day.

  • Will a provider be monitoring my day to day blood sugar readings?
A provider will not be monitoring your day to day blood sugar readings. However, at the beginning of the program, participants will be provided with material and instructions on how to use the CGM device and how to properly read their blood sugar levels.
  • How often should I scan my sensor?
It’s important to scan the sensor every eight hours (at least). Upon scanning, the device will transmit the previous eight hours of data. We recommend that participants book a provider visit ahead of time so they can ask questions and discuss the significance of their readings with a medical professional at the end of the program.
  • Does a CGM hurt?
Continuous glucose monitoring allows you to obtain a complete record of your blood sugar levels without daily finger pricks and with very little to no discomfort.
  • How do I interpret my blood sugar levels?
When you are reading your blood sugar levels, it’s important to look at the overall picture. Variations in your blood sugar levels throughout the day reflect how your body absorbs and stores glucose. People without diabetes typically see their blood sugar levels rise no more than 50 mg/dL with meals, while levels tend to fall below 100 mg/dL while fasting (for instance, overnight). People with prediabetes or diabetes are likely to see higher levels than these. Of course, these ranges and values vary by individual, so you should consult a provider if you have any questions about what your blood sugar levels mean.
